Many approaches and models may be helpful for implementing and assessing collaborative care. Over time this seciton will be expanded to include guides, research data, and other helpful resources.
One approach for implementing collaborative care is described in a book called Field Guide to Collaborative Care, by Paul Uhlig and Ellen Raboin. This book, and a companion set of discussion guides called the Collaborative Care Toolkit, were developed from more than15 years of practice and research about collaborative care across multiple settings. If you are intersted in implementing or understanding more about collaborative care, the Field Guide and Toolkit may be useful to you.
